Our Intake and Enrollment teams guide individuals through determining functional eligibility and ensuring they meet targeting criteria for specific waivers and/or programs for which they are applying.
Once functional eligibility is confirmed, Medicaid and Colorado’s Department of Human Services assess financial and disability qualifications.
Required Documentation for HCBS Waiver and CFC Enrollments
- Professional Medical Information Page (PMIP) — within 30 days of Colorado Intake Screen (CIS)
- Release of Information (ROI) — if applicable
- Encryption Opt In/Out Form
- Birth Certificate
- Social Security Card
- Photo ID or signed Affidavit of Identity (Passport, Driver’s License, or State ID)
- Physician Attestation Form (Consumer-Directed Attendant Support Services (CDASS), In-Home Support Services (IHSS), or Children with Complex Health Needs (CwCHN)) — if applicable
- Additional (if applicable):
- Adoption, Name Change, or Guardianship Paperwork
- Special Needs Trust Approval

CFC is a Medicaid program that expands service and service delivery options for HCBS to Health First Colorado (Medicaid) members who need long-term care, including members who are currently on an HCBS waiver.
