Case Management Redesign (CMRD) consists of several initiatives that will help make accessing long-term services and supports easier by having Case Management Agencies (CMAs) serve all Home and Community-Based Waivers (HCBS) so individuals and their families can easily access the services they need through a single local agency.

Serving Individuals & Families

Meet the Myers and their 5-year-old son Grant, who is on the autism spectrum. Their love and dedication to their son are incredible, and they have some great advice for other parents.

Through local funding, we support individuals, families, and providers serving our community.
