Qualifying for Services
In order to access Developmental Pathways programs, an individual must first be determined to have a developmental delay or developmental disability. Developmental Pathways makes this determination during the intake and eligibility process.

An intake case manager will discuss the criteria for a developmental disability and will provide you with a Request for Determination of Developmental Disability application packet. The intake case manager will also discuss potential programs and supports.
The intake case manager will review your submitted documents and complete an Intellectual and Developmental Disability determination.
Unfortunately, there is currently a waiting list for funding for some of our services. Your placement on the waiting list is determined by the date of the Developmental Disability determination. There may be some immediate access to other supports, and your intake case manager may be able to suggest alternative community resources.
See our Determination of Developmental Disability form and other print materials to get started with the intake and determination process.
