Community First Choice (CFC) is a Colorado Medicaid program that expands access to attendant services such as personal care and homemaker services, allowing more individuals to receive necessary support without requiring enrollment in a Medicaid waiver.
To qualify:
- Eligible for Health First Colorado benefits under the State Plan and meet an institutional level of care. An institutional level of care refers to an individual who has needs that require services in their home and community to keep them from long-term care placement in a nursing home, hospital, intermediate care facility for individuals with intellectual disabilities, or an inpatient mental health facility. Institutional level of care is determined by the state prescribed level of care eligibility assessment. or
- HCBS waiver members who receive one waiver service per month.

How services are approved for children
For children, service approval includes an important additional factor: what is typical for a child’s age.
CFC services are not intended to replace everyday parenting responsibilities. This means services are approved when a child’s needs go beyond what is generally expected for their age and development.
In some cases, services may be approved when a child has documented medical or functional needs that require a higher level of care.
Your case manager will walk through these guidelines with you and help determine what services may be appropriate for your child.
Your case manager may also use tools, such as the Direct Care Services Calculator and Age Appropriate Guidelines, to help determine appropriate services for your situation. If you’re unsure what may be covered, your case manager is the best place to start.
