At Developmental Pathways, we value kindness, collaboration, and competency and commit to person-and-family-centeredness. We strive to create and support an environment of inclusion and belonging for individuals and their families as well as our internal team members and external community partners.

Early Intervention Team

Supporting seven regions, this team supports babies and toddlers with developmental delays and their families with accessing vital Early Intervention supports.

Key pursuits are:

  1. Service coordination and support accessing EI services
  2. Pediatric Home Health Services, including physical therapy, occupational therapy, and speech-language pathology
  3. Transition to preschool supports and/or Part B Services

Case Management Team

Our Case Management team focuses on providing great customer service while assisting the individuals we serve and their families towards meeting their goals and living a meaningful life. Using a person-centered approach, we strive to empower individuals to have input and control of their lives and make safe and informed choices. 

This team helps:

  1. Understand what services are available and determine if you meet the criteria for developmental disability services
  2. With enrolling you in the available waiver or state-funded program that is right for you
  3. Determine the right combination of care, activities, and support needed to live a meaningful life
  4. Develop a service plan and assist you in finding providers and meeting your goals for mental health, emotional, social, and medical services
  5. Ensure your health, safety, and other needs are met and your rights are respected
  6. Monitor your service plan for effectiveness and satisfaction

Systems Navigation Team

Our Systems Navigation teams exist to help individuals and families navigate the complex long-term care system, including I/DD determinations, screening, referrals, and needs assessments. They also offer guidance and assistance with obtaining and maintaining Health First Colorado (Medicaid) benefits.

The Crisis team and County Liaisons are also a part of this team.

Program Quality Team

Our Program Quality team collaborates with our Case Management and Systems Navigation teams to

Community Engagement Team

The Community Engagement team works on building a stronger community by building, improving, and advancing local programming that helps us support a cohesive, connected, and resourced disability community. 

This team also supports the Community Outreach Waitlist Services Program.

  1. Direct funding to individuals and families
  2. Direct funding to providers in support of individuals and families
  3. Agency-level work to strengthen I/DD outcomes for our community
