Nursing facilities (NF) are institutions certified by a state to offer 24-hour medical and skilled nursing care, rehabilitation, or health-related services to individuals.
Medicaid helps cover three different levels of care including 1) Skilled Nursing Care 2) Intermediate Nursing Care and 3) Residential Care. Â

Case Management Agencies assist with referrals to nursing facilities. Â

To qualify:
- Be 65 years or older and/or meet other targeting criteriaÂ
- Meet the functional and level of care requirements which are determined through a designated assessment process.Â
- Meet financial determination requirements as outlined in the Colorado Code of Regulations and as determined by your local Department of Human Services. Â
