The Colorado Department of Health Care Policy & Financing (HCPF) Awarded Developmental Pathways the Case Management Agency Contract for Arapahoe, Douglas, and Elbert Counties

Aurora, CO – (August 14, 2023) – Developmental Pathways (DP), a leading provider of early intervention and case management services, is thrilled to announce its recent success in securing a Case Management Agency (CMA) contract, effective November 1, 2023, with the Colorado Department of Health Care Policy and Financing (HCPF) to provide case management services to individuals enrolled or enrolling into Long-Term Services and Supports (LTSS) for defined service area #5, which includes Arapahoe, Douglas, and Elbert counties. This exciting development marks a significant milestone for the organization and highlights its commitment to delivering exceptional services to a broader range of individuals and families with disabilities.

DP was selected through a competitive process requiring local knowledge and expertise along with a track record of excellence in customer service and innovation.

Developmental Pathways plans to invest in additional resources, including personnel, infrastructure, and technology, to ensure a seamless transition and optimal service delivery for all individuals and families. The organization is confident that this expansion will create new employment opportunities in the community, contribute to better customer service, and improve access to services.

All Adult HCBS waiver programs:

  • Brain Injury Waiver (BI)
  • Community Mental Health Supports Waiver (CMHS)
  • Complementary and Integrative Health Waiver (CIH)
  • Developmental Disabilities Waiver (DD)
  • Elderly, Blind, and Disabled Waiver (EBD)
  • Supported Living Services Waiver (SLS)

All Child HCBS Waiver Programs

  • Children with Life Limiting Illness Waiver (CLLI)
  • Children’s Extensive Support Waiver (CES)
  • Children’s Habilitation Residential Program Waiver (CHRP)
  • Children’s Home and Community Based Services Waiver (CHCBS)

Four Non-HCBS programs

  • Program for the All-Inclusive Care for the Elderly (PACE)
  • Long-Term Home Health (LTHH)
  • Hospital Back-Up (HBU)
  • Nursing Facilities (NF)

As the designated Community Centered Board in their catchment area, DP will continue to provide:

– Early Intervention programming

– Locally funded programming

– Continued support of all state-funded programming for individuals with DD including Family Support Services, State Supported Living Services, and OBRA Specialized Services.
