The Children’s Extensive Supports (CES) waiver helps children and families by providing services for children with a developmental disability who also demonstrate a medical or behavioral condition where constant line-of-sight supervision is regularly needed to keep the child and others safe. There is no State waitlist for this program.
CHRP
Children’s Habilitation Residential Program Medicaid Waiver (CHRP)
Birth Through Age 20
The Children’s Habilitation Residential Program (CHRP) waiver offers services for children and youth who have an intellectual or developmental disability, very high needs or a serious emotional disturbance. Their needs for support put them at risk of, or in need of, out-of-home placement.
CwCHN
Children with Complex Health Needs Waiver (CwCHN)
Birth Through Age 18
The Children with Complex Health Needs (CwCHN) waiver provides support for children with complex health needs and/or life-limiting illnesses, and who are at risk for institutionalization in a hospital or nursing facility. The waiver allows families to continue to seek curative treatment while their child is receiving palliative or hospice care.
CFC
Community First Choice (CFC)
All Ages
Community First Choice (CFC) expands access to attendant services such as personal care and homemaker services allowing more individuals to receive necessary support without requiring enrollment in a Medicaid waiver.
FSSP
Family Support Services Program (FSSP)
Generally under 18 Years of Age
The Family Support Services Program (FSSP) is a state-funded program that provides individualized support to families who are caring for a family member living in the home with developmental disabilities or delays with costs that are beyond those typically experienced by other families.
