Programs for Children Birth to Age 3

Early Intervention

A program for families with a child up to age 3 with a delay in development and/or qualifying diagnosis. We will evaluate your child’s development and determine if your child qualifies for supports and services. Our primary services include speech, occupational, physical therapy, and developmental intervention.

Long-Term Care Programs

Children’s Extensive Support Waiver (Birth to 18 Years)

The Children’s Extensive Supports (CES) waiver helps children and families by providing services for children who have an I/DD who also demonstrate a medical or behavioral condition where constant line-of-sight supervision is regularly needed to keep the child and others safe. There is no State waitlist for this program.

Children’s Habilitation Residential Program Waiver (20 Years or Under)

The Children’s Habilitation Residential Program (CHRP) waiver provides services for children and youth who have an I/DD and complex medical and/or behavioral needs. Their needs for support put them at risk of, or in need of, out-of-home placement.

Children with Complex Health Needs Waiver (Birth through Age 18)

Children with Complex Health Needs (CwCHN) waiver provides support for children with complex health needs and/or life-limiting illnesses, and who are at risk for institutionalization in a hospital or nursing facility. The waiver allows families to continue to seek curative treatment while their child is receiving palliative or hospice care.

Family Support Services Program (Generally Under 18 Years)

The Family Support Services Program (FSSP) is a state-funded program that provides individualized support to families who are caring for a family member living in the home with developmental disabilities or delays with costs that are beyond those typically experienced by other families.

Local Programming

Resource Coordination Program (3 – 18 Years)

The Resource Coordination Program (RCP) provides support to individuals waiting for state or federally funded services; local dollars, grants, and donations provide funding for this program.

Long-Term Care Programs

Brain Injury Waiver (16 Years & Up)

The Brain Injury (BI) waiver helps people with a brain injury who need extra support to live in their communities. You can get these extra services along with your regular Health First Colorado covered services as long as they are not duplicative.

Community Mental Health Supports Waiver

The Community Mental Health Supports (CMHS) waiver provides assistance to people with a mental illness that require long-term supports and services in order to remain in a community setting.

Complementary and Integrative Health Waiver

The Complementary and Integrative Health (CIH) waiver (formerly SCI Waiver) helps people living with a spinal cord injury (traumatic or nontraumatic), multiple sclerosis, brain injury, spina bifida, muscular dystrophy, or cerebral palsy with the inability for independent ambulation over the age of 18.

Developmental Disability Waiver

The Developmental Disabilities (DD) waiver provides access to 24-hour, seven days a week supervision through Residential Habilitation and Day Habilitation Services and Supports. There is a statewide waitlist for this program.

Elderly, Blind, and Disabled Waiver (18–64 Years OR 65 Years & Up)

The Elderly, Blind, or Disabled (EBD) waiver provides assistance to people ages 65 and older who have a functional impairment, or are blind, or to people ages 1864 who are physically disabled or have a diagnosis of HIV or AIDS, and require long-term supports and services in order to remain in a community setting.

Intermediate Care Facility

An Intermediate Care Facility for Individuals with Intellectual Disability (ICF/IID) is a residential facility that is certified by the Centers for Medicare and Medicaid (CMS) to provide habilitative, therapeutic and specialized support services to persons with intellectual and developmental disabilities.

OBRA Specialized Services

OBRA Specialized Services (OBSS) provides integrated community-based supports to individuals with I/DD residing in nursing facilities. Support and services are similar to the S-SLS.

Supported Living Services Waiver

The Supported Living Services (SLS) waiver provides a variety of services for adults with I/DD living in their own homes or with family in the community. This program is not a 24-hour program.

State Supported Living Services Program

The State Supported Living Services Program (S-SLS) program provides a variety of services for adults with I/DD including pre-enrollment into an I/DD waiver and ongoing supports for those who do not qualify for LTC Medicaid.

Programs of All-Inclusive Care For The Elderly

Programs of All-inclusive Care for the Elderly (PACE) provides comprehensive health care services to their participants. One of the main objectives of the PACE program is to enable older adults to live in the community as long as medically and socially feasible.

Hospital Back-Up

Hospital Back-up (HBU) supports qualified skilled nursing facilities in providing hospital level care to clients who are ventilator dependent, have complex wounds, or have medically complex needs.

Long-Term Home Health

The Home Health benefit for Health First Colorado (Colorado’s Medicaid program) members provides services from a licensed and certified Home Health Agency (HHA) for members who need Intermittent Home Health Services.

Nursing Facility (or Skilled Nursing Facility)

Nursing facilities (NF) offer 24-hour medical and skilled nursing care, rehabilitation, or health-related services to individuals through different levels of care.
